Mission Statement
HOPE SERVICES CENTER LLC will provide the highest quality of employment orientation and personal care services to people with disabilities. All our policies, procedures, and practices will reflect our commitment to providing people we serve with the highest quality of services available. This commitment will always reflect on HOPE SERVICES CENTER’s mission.
How We Started
The company started out in the year 2013 and began offering Employment Services to the Department of Vocational Rehabilitation (DVR) clients (DVR vendors). We provide Community Employment Placement and other related DVR services for DVR eligible clients. Eventually, in June 2016, we added HCBS services in the business.
Who We Serve
At HOPE SERVICES CENTER LLC, we serve everyone with intellectual and/or developmental disabilities that has been determined through the various waivers. The Home and Community Based Services (HCBS) waivers offer Health First Colorado members who meet special eligibility criteria access to additional services in their homes and communities as an alternative institutional care. The Home and Community Based Services waivers for people with intellectual and/or developmental disabilities We serve includes:
- Home and Community Based Services for Persons with Developmental Disabilities Waiver (HCBSDD)
- HCBS-Supported Living Services (HCBS-SLS)
- HCBS-Children’s Extensive Support (HCBS- CES)
Persons with Developmental Disabilities Waiver (HCBS-DD)
The HCBS-DD Waiver provides persons with intellectual and/or developmental disabilities access to services and supports 24 hours a day to allow them to live safely and participate in their community. We can assist them as they transition to a residential host home, and we offer personal care services.
Supported Living Services (SLS)
The HCBS-SLS Waiver provides services and supports to assist persons with intellectual and/or developmental disabilities to live in the person’s own home, apartment, family home, or rental unit that qualifies as an SLS setting. We can support them by providing personal care, respite care, supported community connection, enhanced/basic homemaker, and mentorship.
Children’s Extensive Support (CES)
The HCBS-CES Waiver is for children ages birth to 18 with intellectual and/or developmental disabilities or for children ages four (4) and under who are at risk of a developmental delay. We offer respite care, supported community connection, and enhanced/basic homemaker.
